occurs
when
the
thyroid
gland
swells
and
becomes
visible
at the
front
of the
neck
,
often
as a
result
of
iodine
deficiency
.
Crile
described
this
milestone
case
in his
autobiography
,
stating
the
goiter
was
so
extensive
it
had
invaded
much
of the
upper
thorax
and was
extremely
difficult
to
work
with. "
I
needed
all
the
technique
that
I
had
developed
in those
twenty-five
thousand
operations
" he
wrote
.
Dr
.
George
Crile
Jr
.
assisted
and
is
at the
near
left
in the
photograph
.
Although
he
performed
many
kinds
of
surgery
,
George
Crile
Sr
. was
known
for his
skill
in
thyroid
and
goiter
surgery
. In
other
hospitals
,
mortality
was
generally
very
high
in these
delicate
operations
,
mostly
because
of
stress
,
trauma
, and
surgical
shock
.
Crile
devised
a
special
routine
to
avoid
shock
and
trauma
,
known
as "
stealing
the
thyroid.
" For
several
days
before
the
actual
operation
, a
nurse
would
go
through
the
motions
of
preparing
the
patient
for a
real
operation
(even
giving
small
doses
of an
anesthetic)
to
familiarize
the
patient
with the
procedure
.
Unannounced
,
Crile
would
arrive
in the
patient's
room
(to
further
avoid
shock)
and his '
assembly
line
' of
nurses
,
surgeon
,
assistants
, and
others
would
work
so
quickly
that
when
the
patient
woke
up
, he would not
realize
that the
operation
was
already
over
.
George

Sr
.
(1864-1943)
was
born
in
Chili
,
Ohio
, and
received
his
undergraduate
education
at
Ohio
Northern
University
(1885)
, his
medical
degree
from
Wooster
Medical
College
(1887)
, and
additional
training
in
Europe
.
Crile
worked
in
military
hospitals
in
both
the
Spanish-American
War
and
World
War
I
. He was
one
of the
founders
of the
Cleveland
Clinic
Foundation
and
served
as
both
president
(1921-1940)
and
trustee
(1921-1936)
.
