Before the subscriber, a Notary Public, (or Justice of the Peace) [crossed out] Judge of the Probate Court [written above] in and for said county, personally appeared the
above named Mary Clay who, after being duly sworn, says, that the above
statement is true in every material respect; that no payment has been made or compensation received in any way, or
from any source, for the whole or any part of said claim, that the said claim has not been transferred to any person
or persons; that the rates and prices charged are reasonable and just, and do not exceed the market rate or price of
the articles at the time and place stated; and that the following is a true and full statement of all the facts and circum-
stances attending the taking, injury or destroying of the said property.
